with the Iowa Department of Education to help schools assess the health
of the school environment. We hope schools will use the data to develop
local school wellness policies. The information about you or your school
will be used to develop a system to combine school building information
into district information and a statewide profile. The information will
also help us understand the challenges schools face as they develop wellness
policies. Information you provide is important to us.
We want you to answer each question carefully and honestly. Schools will have access to their information and combined information from all the schools; however, no one will be able to identify you or your school in the combined data. You may decide you do not want to finish this survey, that is okay, you may stop anytime simply by logging out of the survey. You may participate in the assessment, but exclude your school’s data. The questions you will be asked are about the practices, policies and procedures in your school. Completing this survey is similar to the work many educators do regularly, therefore there is little or no risk related to completing this study.
